Indeed, I'd expect no less when _you_ set up the files :-}

Here's a cut-down copy of the bookmap file (the full-size one has around 300 
topics), plus one of the files where the <keyref> refuses to resolve.

By the way, the context is that I have two more teams asking me to help migrate 
their content to DITA! one team may even end up deciding to order a licence and 
manage their own files, if the initial project is successful.
