I'm puzzled again :-} by some sort of discrepancy I can't see the reason for ...

I have an ordinary ditamap that includes three <keydef> elements before the 
first <topicref>.

The option 'use as master' document is not selected. The first topic file 
included in the book uses a <keyword> element, as shown, which resolves without 
problems when the map is converted to .pdf

I have a bookmap which includes three <keydef> elements in the <frontmatter>, 
before the generated toc, figurelist and tablelist. The option 'use as master' 
document is selected.

I'm hoping the problem is related to where I have defined the <keydef> 
elements. Let me know if you need to compare the two map files, and thanks in
