_wait_for_vhd_coalesce doesn't kill image when exception raised

Bug description:
If we exceed the max number of attempts to poll for a VHD, it should
kill the pending image in glance.
As of right now, that image will hang in the pending state forever.
We should also revert the vm_state and task_state on the instance
along with this.
