build_instance does not recover from _setup_block_device_mapping

Opinion
Bug description:
It appears that if any of the block device mappings fail to be created
or attached that the whole instance as a whole will be failed, this
does not allow for graceful retry or recovery from said failure but
instead forces the instance to ERROR (which then requires manually
recovery).
