Security Group Add Rule form is not setup properly.
We can see only "Rule" and "Remote" fields.
When I change a selected item in "Rule" to others, all expected fields were
displayed.
After that when I selected "Custom TCP Rule" again, all expected fields were
displayed.
>From this, it seems all fields are prepared expectedly but the initial setup
