I may have fixed the problem by running Applejack v 1.5 (restart holding
down the Cmd and S keys). Since Applejack did its thing, I have not seen the
problem and I have restarted Entourage several times.

Applejack <http://www.macupdate.com/info.php/id/15667/applejack> is one
of the best kept secrets for OS X.  It does nothing until you need it and
when thing start acting weird, just restart into Applejack and enter the
command 'applejack auto restart' and it does its thing.

In this case I have no idea way Entourage was crashing and sending reports
back to the Microsoft.  I think Applejack may have cleared the problem that
rebuilding permissions failed to correct.
