When I did it it was straight forward.  If the OID was working (i'd verify 
using snmp walk).  I'd add the data sources, then add thresholds and graphs.  
It tends to take a little bit to get data in the graph, but you can run 
zenperfsnmp from the command line and grep the output for errors.  Somewhere in 
the output you should see it polling the OIDs on your devices, if you don't 
then you should check your data sources.

If all that works, give your graph some time to populate.  If I can figure out 
how, I can try to turn mine into a zenpack so you can see how I configured 
things.
