The documentation for makehosteddomains lead me astray — my mis-reading of it — I’d read the following and thought it implied running makehosteddomains made the changes live:

> The makehosteddomains script must be run in order for any changes to/etc/courier/hosteddomains to take effect.

I can see how it can be interpreted this way.

I'll make this wording more clear.
