Thank you Leonard, you have actually given me some ideas for my next 
step/troubleshooting process.  See responses to individual questions below:

Distillation is the process of converting Postscript to PDF.  Is that really 
what you are doing using the Acrobat SDK?  If so, please explain what version 
of Acrobat/Distiller and the workflow.
[Rick.Wellman] Yes, that is what we are doing.  I left out that we are actually 
using Acrobat SDK to convert/save as Postscript first.  A logical next question 
might be:  Why are you doing that?  The best/short answer I can give you is 
that we have another 'Vendor Supplied' PDF tool that is buggy and we have found 
through trial and error that this seems to prevent THAT particular bug.

If you are converting Postscript, as you imply, then anything is possible since 
it's a full fledged programming language - and one that iText knows nothing 
about.  You'd need to read the Postscript and find things like loops, memory 
allocators, etc.
[Rick.Wellman] Thanks.  This gives me some ideas on how to investigate more!

If yoga re talking about something completely different, then please explain 
EXACTLY what you are doing.

Also, without seeing actual PDF files, anything we say will be a COMPLETE WILD 
A** GUESS.
[Rick.Wellman] This is a well understood and valid point.  I would have done 
that in almost any circumstance but the data in the document is sensitive and 
covered under contractual obligations.  I certainly wish I could.
