Adobe Reader makes thumbnails automatically
(open the Pages panel to the left). You can use iText
to change these thumbnails and the page labels that
are shown under the thumbnails.

You can use iText to import pages from existing
PDF documents and use them as thumbnails in
other PDF documents.

It is not possible to convert a PDF page to an image
files (JPG, PNG, GIF,...) with iText. So you can't
create images that can be used as a thumbnail with
iText.
