Leonard is right, but nobody will blame you for being confused. Please go to page 202 and read the part entitled "Page numbers versus page references". That explains the cause of your confusion.

Page 202 show an output that looks like this: AR=[210 0 R, /XYZ, 36, 802, 0].

"210 0 R" is a reference to the object with number 210 which is the page dictionary for page 19. How do we know that object 210 corresponds with page 19? There are no page numbers inside a PDF. The page number of the page in object 210 depends on the order of the elements in the page tree.

If some methods allow you to choose a page number (to create a bookmark) or to get a page number (when retrieving info about a bookmark), iText has translated to object reference into a page number after inspecting the branches of the page tree.
