General settings has no effect whether its enabled or not.  I can't seem
to find the second setting you were talking about.  Can you point it out
to me?
Remember I am still using .17.  this feature may have popped in at .18
or so.

There is an option in the mythtv general settings to disable it
controlling sound levels I believe... and another if you are using the
pvr-350 output to tell it you aren't looping sound through the sound
card. If you disable both of these I don't think it will mess with
sound at all.
