here are parts of my lyx-files:
the one of your attached lyx-file superscript...

\begin{eqnarray*}
 & \sum ^{n}_{i=1}i & \\
 & \sum\nolimits _{i=1}^{n}i & \\
 & \int\limits ^{\infty }_{x=0}e^{-x^{2}} & \\
 & \int ^{\infty }_{x=0}e^{-x^{2}} & 
\end{eqnarray*}

it's just the same as you described above and
it's right, that the second sum has no real super/subscript.
But when I delete the sum-symbol and write it again, it always
works well. 
My lyx-file is (only the interesting part):

\begin{eqnarray*}
 & \sum ^{n}_{i=1}i & \\
 & \sum ^{n}_{i=1}i & \\
 & \int\limits ^{\infty }_{x=0}e^{-x^{2}} & \\
 & \int ^{\infty }_{x=0}e^{-x^{2}} & 
\end{eqnarray*}

I have no idea what's wrong with your LyX-Version.
