Fermi - Similar to Callaway.  Basically if a source is "assigned" to an
area (cal shop / count room) it may be used with any additional paperwork.
If sources need to be used by personnel other than RP / Chem techs, they
are issued to that individual (this is very rare occurrence, nearly all
source use is by Chem / RP techs only)

      How do you handle a source which is stored in the same room in which
      it is used?  Once it is removed from its lock box / storage
      container / cabinet for use,  do you
      (1) consider that source “issued” to that person? No,
      (2) require that it be logged in/out using a form or some electronic
      process? No

      Does your source control procedure (or other controlling document)
            (1)  contain an issue log or mechanism to track a sources
            removal from a storage location? yes
            (2)  Provide detailed instructions on what actions constitute
            an “issue for use”? yes
            (3)  Provide an exemption for the logging of sources which
            never leave the room in which they are used? yes

A source in a room taken out of the locker for use (example count room) is
NOT an "issue" or "movement". The technician is responsible for the use.
One caveat is IF the source is outside the Protected Area would require to
be Locke or guarded. (I don't mind if the technician leaves the count room
for lunch or tasks if a source is on a detector for example.)

We would not log such use.

Our program does not define "issue for use". Source checkout involves a
manual form at the locker, unless it's a high activity source that could
require RP posting control.

Our program requires a checkout form at the locker with database update for
movement.   Movement would only apply locker to locker.
