Hi Jason,
at the moment this is not possible, as there is no metametadata.
All the metadata at the moment is associated with the item. Your usecase
would need an item with the metadata dc.creator.author and the metadata
dc.creator.author with the metadata dc.date.dateofbirth. With the evolving
architecture this should be possible in the future, i.e. having metadata
for each DSpace object.

I was wondering if it was possible to extend the various input-type
choices available within input-forms.xml. I'm extending the UI for a
Theses and Dissertations collection, and I need to be able to tie a date
of birth to each author. I can add another repeatable date for
dc.dateofbirth, but ideally, it'd be great if I could add a new
input-type of theses-author which contained a family name and first
name field, as well as a date field.
Is this possible within the current frame work? Will I need to do this
by extending the inner workings instead?
