You nailed it. Turns out I had Sun's 1.6 on the system,
but Ubuntu was pointing to gcj. I used update-alternatives
to select 1.6, and that took care of the problem. Everything
is back to the expected speed.

That will render without any customization of DocBook or the
stylesheets. Then, if you want parameters to look different,
you can customize the parameter element in your xsl, which
is much easier than adding a new list element.

0.93 does generate hyperlinks in your pdf index.
Incidentally, I've found 0.93 to be very useful. It won't
do everything the XEP or Antenna House can do, but I did
find that it's a lot better than 0.20.5 and is definitely
worth a look if you can't use a commercial processor.
