Hmm, I just looked at my copy of the template pack and the square behind the chapter number is missing. How annoying. The good news is that if you click File > Document > New and then click Explore Standard Templates at the bottom of the page, there's one with the color block under User Guide > Chapter. You should be able to replace that color block with your customer's graphic. If you put an appropriately sized version of it on a reference page, you should be able to bring it in with Frame Above or Frame Below paragraph for the section numbers and page numbers. If no one contacts you, try the Adobe Template Pack 1, Book 7 style. That has a background image for the chapter number and I'm sure you could adapt that easily.
