how to itemize exercises?

hi all, I want to use Theorems module to itemize excercises, please refer to attachment for a test file. But when preview with pdf file, the exercises are not itemized, just the first exercise have the prefix number, I don't know what I'm missing. Thanks in advance for any help! -- Best
