in the documentation but not where I looked. This probably relates to the
recent discussion regarding manuals for novices.
My observation is that \completecontent and \completeindex are treated like
chapter starts in that

wanted to ask is why \complete... use sometimes
\title to place the header text (e.g. for \completecontent) and
sometimes \chapter (e.g. \completeindex) to place the header text. For
bibliography this is even weirder because MKII use \systemsuppliedtitle
while MkIV use \systemsuppliedchapter.

If your question is of interest to
upregister[index][indicator=yes] does not help.
Another, probably unrelated, problem is that \completeindex does not
seem to do anything.
Can you add

Could someone point me to documentation that describes how to change the
format of the index created by \placeindex and \completeindex?
In particular, I want to get rid of the entries created for the letters of
the alphabet because I find them an unnecessary use of space.
