Try connecting with the SSH packets and raw data option enabled in the PuTTY
logging screen. (don't forget the log file name)
That should get you
systems all show SSH-2.0.

Smells like a Windows firewall issue.
I'd open up port 22 outgoing from the Windows machine to the various
network gear to which you're trying to connect.

1. It connects for a few seconds, displaying “SSH-1.99-Cisco-1.25”.
I never get a login prompt. Any keystroke (or doing nothing for about 5
seconds) results in “Connection to host lost.)

Which version of puTTY are you using?
If 0.62, try that version.
If it's 0.62, does the latest development snapshot make any difference?
I vaguely remember having similar sorts of issues with older versions of puTTY
on Windows 2K8 R2
