Which version schould be running on production systems?
Is 5.4.2 only a bugfix-release or really a development-release?

with all the security and recommended patches. But I keep on getting this

Config.status reports that the roaming option is enabled...yet my smtp.tcp

You will notice the message has come from which means its likely a bounce

Without turning on more logging it's a little hard to say for certain.

What you say may be true. But I can tell you from our experience that
Eudora cannot smtp authenticate unless cram-md5 is 'undef' in qmail-smtpd.c
whereas Outlook can.
