I would like the approach used in VMWare:
When the mouse pointer is within the virtual machine's screen (that is,
not only is the window active, it's having the mouse pointer within its
zone) all ALT and Ctrl combinaisons (except Alt+Ctrl+Del) are
transmitted to the virtual machine.
