I suggest you look at ftp://publisher.libronix.com/drop/Tiro/SBLHebrew-Distribution/SBLHebrew-Manual.pdf (prepared by John Hudson), section "The normalisation issue" and Appendix B, and at http://scripts.sil.org/cms/sites/nrsi/media/BibHebAltCharsProposal.pdf (prepared by Peter Constable). These both explain the problem in some detail. They also propose alternative combining classes for the Hebrew vowels without actually proposing that the existing Unicode definitions should be changed.
