According to the documentation, "By default, the template is empty and
connections from all hosts are accepted."  I would interpret that to mean
that unless the connect is specifically denied (or query) that it is
accepted.  That is why all the examples of only allowing one IP start with
"-:" first.  As an example "-:+10.0.60.101" specifically denies all
connections, then allows 10.0.60.101.
