I'm not sure about combining the cards into one, but you can get around the card ordering problem by using the cards' names instead of numbers. Look in /proc/asound/cards, it will list the cards' names in square brackets eg on my system: "1 [M44]: ICE1712 - M Audio Delta 44 M Audio Delta 44 at 0xcf00, irq 20" where M44 is card name.
