+...+ for chords is obsolete long, long ago.

That's probably because of the infamous line-breaking daemon. 
M:3/4 without brackets mid-line has never been supported by any 
abc application as far as I know.

Also blame the infamous daemon! Much of the abc available on 
the web has been sent through e-mail at some time. You just 
have to have a user option for when to relayout line breaks.
