I suppose, While one cannot do anything regarding a password protected file,
the screen reader certainly does have limitations in connection with a
protected layout documents, in which case, a sighted person is completely
able to read the contents but no screen reader would read it.

The most obvious problem arises when the pdf file is password protected
and one can read the file but cannot modify. In such cases, unless one
knows the password, there is no solution. Bank statements, creditcard
bills etc come in this form.
Second problem arises when some takes photo of some written pages and
saves the image in pdf form. In such cases, Jaws command "insert+space +O
W" is very helpful but it is not the solution.
Third major problem is related to scanning. While scanning a document,
various unnecessary characters hamper the reading the document.
Especially, CBSE books display the title of the chapter on every page.
Hence, when we give a read all command, each page begins with the chapter
name. some times, this chapter name comes in between a sentence, resulting
in misleading meanings.
While reading maths and science books, tables and charts are not properly
read.

Considering the pain of blind professionals and students, some of us have
decided to work with Adobe and Vispero to find out ways in which PDFs can
be made better for people with blindness. And I am doing everything
possible to get this to Shantanu’s (CEO of Adobe) attention, so that the
bottlenecks between business, legal, compliance and development can be
adressed. The good thing is that the head of accessibility at Adobe and
Vispero have been very receptive.
While the major problem of inaccessibility of PDF is because a lot of
things are dependent on authors for ensuring accessibility, but this
approach itself needs a relook. Changing millions of minds is difficult,
but creating ways to empower millions is certainly possible and the
technologies meant to assist people with disabilities is a real example of
this.
