Can you turn on MPT_DEBUG_INIT in drivers/message/fusion/mptbase.h,
i.e., add a line like
#define MPT_DEBUG_INIT 1
at the beginning of the file? That should turn on some debug output
when the driver claims and initializes the device. When it doesn't
work, you're seeing:

which suggests that mpt_attach() isn't even being called. The first
thing mpt_attach() does is call pci_enable_device(), which is what prints the
"ACPI: PCI Interrupt 0000:0a:06.0[A] -> GSI 30 (level, low) -> IRQ 18"
message.
Can you capture the failing dmesg with a serial console or netconsole?
