Check to see whether you have "Block Policy Inheritance" set for the Domain Controllers container. If you have, you need to unset it (i.e. allow inheritance) in order for the Account Lockout policy to work.
Account policies are set in the Default Domain Policy, but they are actually enforced by the Domain Controllers. This makes sense given that the DCs handle domain logons. If you set Account policies but block inheritance on the Domain Controllers container, these policies will not be enforced.
