Sadly, we are in the same situation - many of our policies are outdated, some exceedingly so. We have a 3 year cycle on the majority of our policies. We send out notifications, but don't always get responses so they just stay in the database like that. Recently, our system administrative policy person has been giving people deadlines for response - if an author doesn't respond to her by x date, she automatically renews the policy without changes for 3 more years. I work with system clinical/patient care policies, and am starting to crack down on some of our outdated stuff, but it is tough going when one is working with 24 hospitals in 7 different states.
