We've not been successful getting telework here, but we've researched it a
couple times and found the best role models (from our view) are University
of Arizona and University of Michigan.  So based just on our review, here's
the materials we used to evaluate.

http://www.hr.arizona.edu/files/TeleEmpGuidelines.pdf   These are the
guidelines.

University of Michigan requires a signed agreement that is very
comprehensive:  http://www.hr.umich.edu/worklife/docs/teleagree.pdf

Michigan State also has a comprehensive program -
http://www.hr.msu.edu/telecommuting/  and they specifically bring in the
ADA aspect.
