Eliza, I assume that when you talk about registration, you mean that in the sense that you are tracking programs that involve minors, not the registration of the actual participants in a program.
At Purdue, programs for minors can be sponsored by just about any department or entity, and the steps to get the program set up and get participants registered varies. However, any program that involves minors must complete an online registration and certification form, which you can view here: https://purdue.qualtrics.com/jfe/form/SV_cDbV5YspZgcOCjz. We used an online survey tool called Qualtrics to create the form. Qualtrics is the company our university contracts with (as opposed to something like Survey Monkey). This registration process has only been in place since the beginning of this year, so I can tell you that setting up the survey was really easy, but I can't yet speak to the ease of use in reviewing/auditing the collected data.
