Ann, We also do not have a formal policy or procedure for this. We rely on the Responsible Executive, who will simply write us an email, asking us to rescind the policy. We keep this on file. We will not rescind or retire a policy without Responsible Executive direction.

We have a policy right now that has been superseded by a recent revision to our Board policies, and is no longer needed. Unfortunately, I have no official policy/procedure in place to do so.
