Actually we're a small shop and everyone has to get their hands dirty in
support, so everyone has an elevated Domain Admin account for when things
need doing. But you're right, it does give me nightmares. Especially when
they just log in and decide to plaster on a load of patches outside of
normal patch cycles just because "they were there"

Sigh...
