We have something similar happening with some of our dual monitors.  If 
the monitor window is in the secondary display window, i.e. Display 2, 
then any Premiere Pro monitors will not show up.  But, as soon as they 
are moved to the primary display window, the monitors show up.  Does 
this sound like what's happening to you?
