I have never been happy with the quality of Premiere's "Maintain pitch" 
option on audio files.  You may be well served using something like 
Sound Forge, (Audition may be able to do it too, but I use SF) to change 
the speed, then modify the pitch.  Another option would be to use ACID 
or other loop based program.  A primary component of these is changing 
the pitch independent of the speed of clips.  There may also be 
something unique (wild guess here) to the specific file you have and the 
percentage you are changing it.  Try modifying the speed a little (like 
90.75%) and see if it makes a difference.
