I believe it is the device driver which actually causes the entry to be made
to the AIX error log. The absence of error log entries makes me suspect
that the wrong device driver or level of device driver is dealing with the
drive, which can cause odd behavior and its inability to make enough sense
(no pun intended) of status coming back that it doesn't log as an error.
I would do a manual 'cfgmgr' to begin with, as that will write stderr output
about problems it finds. Also check your device driver that is in use in
AIX and see if it is appropriate. Just some thought of things to try.
