I got a work-around ...

First upgrade from 3.7.4 to 4.1.1 and upgradedb works fine
Then, upgrade from 4.1.1 to 4.2.1 and everything is OK

Got the same problem with an AIX server. 

Their recommendation is to go back to 4.1.1 (failing that, back
to 3.7.4)
