This is probably clear enough from the pictures, but you don't connect anything to that waveguide fitting.  The devices butt together and clamp to each other.  Your adapter would have to start with a plate as big as the radio head, and yeah probably a clamp to hold that to the tower.

It might not be obvious to your customer, but Ceragon/Cambium already sells the stuff they would need for that.  There's a remote radio head mount, and a flexible waveguide adapter.  Add a waveguide to coax adpater and appropriate coax cable and they'd have it done.  Obviously there are losses introduced, and none of that stuff is cheap.  If they're upgrading an AF11 they'd save the trouble of hanging a new antenna, but they'll pay big bucks for the time saved.

I have had a request to make a connectorizing kit to allow these radios to be used with AF11X antennas. Not sure it is economically feasible.  Especially if I have to add a way to hang the radio as well.

Sort of a rectangular waveguide opening with a round gasket around it.  Here's a 2+0 adapter to give you the idea.
