The only time I saw a problem was when the landlord was itemizing and
charging for it separately. I've seen it done that way twice, and both
times they had the exact problems you describe.
Just like electric and gas, if they just say it's included but don't
specify the cost then people don't usually have an issue.
Or like someone was getting at....if you charge $20/unit and the
landlord raises the rent $40, then it's a profit center for them and
they have incentive to keep everybody on board.

In that case, even if they had an intractable tenant who wouldn't let
the issue go, they could produce an invoice from you showing that the
cost is only $20/month and then only discount the guy $20. Then they
still win.
