I agree with Geoffrey, except of course when renal failure is to an
extent that they are on dialysis.
We do have what is known locally as the "Mucomyst Protocol". Ref
"Prevention of Radiographic-Contrast-Agent - Induced Reductions in Renal
Function by Acetylcystein" N Engl J Med 2000; vol:343, No:3, 180-4.
This protocol 'appears' to significantly lower the risk in a group of
these high risk patients.

all patients that receive iv contrast media should have their creatinine
and urea checked (if available) before they are given iv contrast. We
are
responsible because we are giving the contrast to the patient.
