I also argue that disabling the Save operation when the document is not modified
is a usability defect.

There are many good reasons for never disabling the Save action even when the
document is not "modified".  For instance, view data may have changed, which
does not necessarily trigger a modified state but the view data get saved with
the document.  Here, the term "view data" includes such things as the cursor
location, zoom level, current active sheet (in Calc), etc.

When looking at applications on Windows, in almost all Microsoft applications
including Word, Excel, PowerPoint, Visual Studio, the Save operation is always
enabled.  Non-Microsoft applications that I know of (such as AutoCAD) also
follows this behavior.  A colleague of mine checked a few Mac applications and
the trend was the same.  So, even from the competitive analysis point of view,
it makes sense to always enable Save.
