I changed the internal cable that went from the SCSI header on the mobo
to the back of the case.  I made sure the cable from the changer had a
good snug connection and that there was no stress on it (it's a pretty
heavy cable with a SCSI-2 to SCSI-3 adapter on it).

I'm running amflush now so we'll see how it goes.
