Start amavisd manually: amavisd debug and see how far does it get. If the 'Creating db in ...' is the last line, then it appears that your installation of Berkeley DB library is broken. If it also reports 'SpamControl: initializing Mail::SpamAssassin' but stops there, there must be something broken in SA.
