First, it's not a case sensitivity assumption about the server, but about
URLs. The specs are quite explicit, URLs are always and everywhere case
sensitive. If a machine wants to return the same resource for two different
URLs, that's up to it, but it doesn't make the URLs case insensitive:
they're not the same URL.
OK, as for hyperlinking it, that's a PAGEINCLUDE question. Maybe *.HTM and
*.HTML should count as pages too.
