This happens on every line -- not just the index.html ones. Are you
suggesting I make an alias for each and every page on my site?
Is this a log format problem because there is no space after the ','
immediately following the .html?

You can use the FILEALIAS command to combine these:
FILEALIAS /index.html* /index.html
