Cancelling and re-ordering does seem to do it. I've told several
people now to re-order, and as they've lost their right to cancel (at
least I presume they have) I say that should they decide they don't
want the app then I'll refund their purchase.

I see people cancelling and re-ordering from time to time, a lot of
very rapid cancellations (within a few minutes) which looks odd to me,
and about 10% 'payment declined: buyer contacted'. I don't think
Market is a very reliable beast...
