Many mobile VoIP  systems are carrier neutral so they can switch between 
GSM/HSDPA/802.11 without too many problems. So I'll expand my question a 
little;

Is there a difference between Hotspot calling and a VoIP client which 
will intelligently switch between carrier technologies 
(802.11/HSDPA/GSM) and use a national call free VoIP provider (similar 
to the 3 skype phone - 
http://www.three.co.uk/personal/products_services_/skype_phone.omp) or 
is there something that I've missed?

If I've not missed anything then it's certainly possible, if there's 
something more funky going on (like the ability to perform the switch 
between a standard voice call over the carrier to a VoIP call whilst the 
call is in progress), then it may be a bit more difficult.
