A link to email where you prefill the subject with app
name/version/phone type/etc. can make it more clear what app/phone/OS
the crash is happening on. Throwing a log in as the body of the email
makes it even easier to figure out what happened. :)

It's not. Many are convinced that's what the ratings are for.
I haven't published my first app yet, I would hope that short of
giving a
crappy review, a user would try to reach out to the developer before
leaving a bad review and rating.
