This may be caused by impromper settings of cache expiration.... I think I
am viewing lot of online pages in offline mode.

There can be more reasons for that, but maybe your CACHE.IDX is corrupted
or not present or the file is on the disk, but not in CACHE.IDX, or
whatever.
